        Post Hospitalization

        Continuity of care exams following hospital discharge can usually be scheduled for the next day. Call us the day of your discharge.

        We have a limited number of open spaces on our daily schedule. Single issue walk-in and same day appointments are accepted based on availability and the immediate need of the patient. Wait times may be longer as scheduled patients will be seen first.
